YOU ARE RESPONSIBLE FOR INFORMING US OF ANY INSURANCE COVERAGE CHANGES FOR YOU AND YOUR BABY

GETTING READY FOR AN OFFICE CONSULT

  • We will have an electronic scribing service for every consult to ensure optimal health record keeping and the expediency of delivery of care plans and physician reports.

  • We are a teaching practice and during most consults we will have a new IBCLC or an intern present observing and participating. If you are not comfortable with a student participating, you must indicate it during the booking process. We reserve the right to reschedule your appointment based on availability with no students.

  • The intake form MUST BE completed in its ENTIRETY and submitted within 24 hours after booking an initial consult and by 10 AM the day before all follow up consults. All Monday consults require a completed intake form by 10 AM the previous Friday.

  • You MUST complete our sickness policy screening the morning of your scheduled appointment.

  • If you have any symptoms prior to the consult, you MUST inform us ASAP and your appointment will be moved to telehealth.

  • At this time only the lactating parent, one support person, and the baby/babies are permitted.

  • At this time masks are optional. We reserve the right to change our mask policy.

  • Baby should be fed following hunger cues. If baby gets hungry the hour before the scheduled time give a small comfort feed or a small snack if using a bottle. Please do not give a full meal within the hour or less before the consult or I will not be able to properly assess feeding.  

  • We will spend a lot of time together make sure that you have eaten and hydrated well prior to your appointment. You should have a bottle of water for yourself.

WHAT TO BRING WITH YOU

  • Make sure you have the pump you are using cleaned and fully assembled including attached bottles. Bring all flange sizes that you have. If you have a Spectra, Motif, Baby Buddha, or Dr. Brown pump only bring the bottles, flanges and the duckbills. For Medela Pump In Style, bring your tubes, flanges, valves and the bottles attached to the flanges.

  • Dress baby in clothes that are easy to remove for weight check.

  • You must bring 2 receiving blankets.

  • Your baby will need to be undressed down to a clean diaper and will be weighed before you start feeding. The baby must remain in just a diaper during the feeding and post feeding weight check. We will use one of the blankets you bring to wrap baby during the feed.

  • Bring any feeding aides, such as nipple shields, shells, hydrogel pads, or others.

  • If you are supplementing baby bring a bottle with milk.  

  • If you are not supplementing but baby has either been actively losing weight or not effectively gaining weight, you must bring a bottle and either human milk or formula in the event that baby needs to be supplemented.

  • Write down all the questions you have.  It can be difficult to get all the work and education done in one single visit.

  • Parking: Meter 2 hour street parking. If you are not a Hoboken resident, make sure to only park on the side with WHITE SIGNS! Parking garage at 215 Hudson Street.

WHAT TO EXPECT FROM US

  • An organic, unscented sanitizer is available so you can clean your hands when you arrive and at any additional time that you may want to.

  • We monitor ourselves for any symptoms of illness on a daily basis.

  • If we have any symptoms prior to our appointment, we will notify you immediately and will move our consultation to a tele-health appointment.

  • We sanitize all equipment, including any computer, phone or tablet we use during our consult, prior to your arrival and after each client leaves.

  • We run a high quality air filter all day while seeing patients and will set it to its highest setting for 30 minutes prior to your arrival.

  • We sanitize our hands throughout the consult.

  • We wear gloves any time we are doing any oral assessment on your baby.

  • We use a clean, new, disposable chuck pad on the sanitized changing table.

  • We use your own baby blanket on the sanitized scale before we weigh your baby.

  • We do not smoke, or wear any perfumes or scented body products. 

  • We will provide you with a written care plan via a secure client portal on the same day as your consult.

  • We do not text and do not use regular email to make sure that your private information is kept secure.

  • Once you book your appointment all communication must take place through the secure client portal. Make sure to check your junk mail if you do not see the invitation to the portal and once you accept it save the email and password for ongoing access.

INSURANCE BILLING

YOU ARE RESPONSIBLE FOR INFORMING US OF ANY INSURANCE COVERAGE CHANGES FOR YOU AND YOUR BABY

IT IS YOUR RESPONSIBILITY TO CHECK WITH YOUR INSURANCE COMPANY IF YOU ARE COVERED FOR LACTATION SERVICES. CHECK UNDER NPI 1457969412 OR 1710273859. GET A CALL REFERENCE NUMBER AND WRITE DOWN THE DATE.

ONCE YOUR BABY IS BORN, WE HAVE TWO CLIENTS THAT WE PROVIDE SERVICES FOR AND WE BILL FOR BOTH FOR EACH CONSULT. ANY CONSULT THAT INVOLVES ASSESSMENT, RECOMMENDATIONS AND A CARE PLAN FOR YOUR BABY AND YOU WILL BE BILLED TO YOUR INSURANCE FOR EACH OF YOU.

AETNA MEMBERS/CLIENTS

Even if Aetna tells you that you are covered 100 percent, Aetna has drastically changed their lactation coverage since March 2024. They no longer cover six consults. They now cover six codes, TOTAL! Each consult is billed as two codes which means you, the parent, will have a total of three consults. Every “free” class you took online has deducted from your allowed of six TOTAL codes per year. They are no longer covering your child(ren)’s services as preventive, which means that their portion of the consult will go towards deductible or if they have met their deductible it will incur a small cost-share. Any ‘Patient Responsibility’ amount will be charged to the card you have on file. If your card is not valid, you will be notified and if the invoice is not paid, there will be a $100 processing fee added and your account will be forwarded for collection services.

If only your baby is covered under Aetna and you, the lactating parent, are not covered under Aetna, you MUST book a self-pay consult. If your baby is not on the lactating parent’s Aetna plan you must book the baby not on plan appointment and you must pre-pay the appropriate fee for each appointment.

We do not offer tele-health for Aetna clients.

CIGNA MEMBERS/CLIENTS

If your baby is not under your Cigna plan, you will have to pay a fee for their portion of the services for every consult. If only your baby is covered under Cigna, you will need to book and pay Self-Pay.

ANTHEM + BCBS MEMBERS/CLIENTS

It is your responsibility to check with Lactation Network to ensure that you are covered for lactation services. Every prenatal consult has a $35 booking convenience fee and every initial after birth consult has a $75 booking convenience fee both of which are due at the time of booking.

TELE-HEALTH DISCLAIMER

We only offer tele-health for Cigna, UHC, Self-Pay and Anthem + BCBS members approved through The Lactation Network. All tele-health consults are thirty minutes.

There are inherent limitations because a digital suckling assessment and a physical breast/chest can not be completed. Potential risks to the technology used for telehealth, are beyond the control of the provider, including interruption of connectivity, unauthorized access, and other technical difficulties.

CANCELLATION POLICY

Appointments canceled with less than 48 hour notice will be charged the FULL consult fee which is not covered by insurance. ALL fees for services are non-refundable and expire 60 days after the original purchase. The payment and no refund policy is SEPARATE from the cancellation policy. Refunds will NOT be issued once you book. Each client is responsible for all fees associated with each consult. Payment is charged at the time the appointment is scheduled. By scheduling your consult, you grant me permission to communicate with your insurance company regarding the services provided to you and to your child(ren).

Rental agreements: no refunds will be issued for any unused part of the rental terms. All equipment must be returned the day prior to the scheduled new charge in order to avoid being charged for a new rental term.
